APPOINMENTS

SCHEDULING A PATIENT

There are two ways for referring practices throughout Arizona to schedule patients with Pain, Spine & Rehab's interventional pain management specialist:

Provide your patient with our contact information and Web site address, painspinerehab.com to schedule an appointment.
Provide our office with the patient's contact information and we will contact the patient to schedule an appointment

In order to ensure your patient has the best possible initial visit with one of our board-certified pain management specialist, we request the following information be sent to the appropriate office via fax prior to the appointment being scheduled:

A Referral Form, including:

Patient's current contact information
Patient's insurance information

Any records the patient may have from your office or a previous pain physician, including  any past imaging records that may be applicable
Insurance authorization if required

Please ask your patients to bring the completed New Patient Packet with them to their initial visit to Valley Pain Consultants.

We accept cash and most health insurances:

• Blue Cross Blue Shield

• Work compensation

• Most commercial insurances
